CMC receives $20,000 diabetes grant

The CVS Health Foundation and National Association of Free and Charitable Clinics (NAFC) recently awarded the Community Medical Clinic of Kershaw County (CMCKC) a $20,000 grant in order to help combat diabetes in the community.

Diabetes has reached epidemic proportions nationally, and the lower income residents of Camden have been disproportionately affected by this chronic disease, CMCKC CEO Susan Witkowski said.

“We’re so thankful to receive this grant, as it is helping us expand our health coaching services and has given us the ability to provide additional equipment and medical resources for our uninsured patients with diabetes,” said Susan Witkowski, CEO, Community Medical Clinic.

The funding will allow the clinic to provide health coaching and case management to 150 uninsured diabetic residents of Kershaw County. The grant will allow the clinic to bring on a nurse practitioner, who will work with the patients as a health coach to support them on their journeys to living a healthier life, with the goal of more diabetic patients improving in at least one clinical indicator.
